Rates
$150-$225 for the initial evaluation. $75-$135 for follow-up visits. Cost is determined by services provided and what is allowed by your insurance carrier.
Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions about physical therapy coverage:
Do I have physical therapy insurance benefits?
What is my deductible and has it been met?
How many sessions per year does my health insurance cover?
What is the coverage amount per physical therapy session?
Is approval required from my primary care physician?
What is my co-pay or co-insurance for physical therapy?
Concord Physical Therapy is a preferred provider for most private PPO insurance companies, Worker's Compensation, and auto insurance as well as Medicare advantage plans through United Health Care. We are not providers for traditional Medicare or Medi-cal plans or HMO plans.
We also accept individuals for self-pay who do not currently have insurance, or have insurance that does not cover physical therapy.
Payment
Cash, check and all major credit cards are accepted for payment.
Cancellation Policy
If you do not show up for your scheduled appointment and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session.
